The Hawaii Diet Manual is used as a reference for diet therapy throughout the state. It is distinguished by its unique cultural perspective which it owes to the state's greatest asset: her people. The Hawaii Diet Manual - 8th edition, released in 2007, is available for sale. 

PROCEDURES FOR PURCHASE

INDIVIDUAL Order Form
This form is for HDA members ($40) and non-HDA members ($55) buying the Diet Manual for personal use.

HDA membership status will be verified by matching the buyer's name with the ADA Data Management Information System (DMIS) list, which is based on current membership renewal.  HDA members are allowed to purchase two manuals for personal use at $40 each.  Non-members can purchase any number for $55 each.

INSTITUTIONAL Order Form
This form is for all worksite Diet Manual orders.  The price for worksite orders is $55 per manual.

• All payments must be cleared by the bank and HDA Treasurer before pick up or distribution of the manual(s) will be made.  Please read the form's payment instructions carefully.
• Oahu buyers: When your payment is cleared, you will be contacted by a committee member to make arrangement for pick up or distribution.  Oahu orders will not be mailed.  Allow 4-6 weeks for processing and distribution of orders.
• Neighbor Island and out-of-state buyers: Shipping and handling fees are added to the purchase price.  Amounts are indicated on the order forms.  Allow 4-6 weeks for processing and delivery.
• A sales receipt will be given at the time of pick up or distribution.
